If you would like to return merchandise purchased from the IDEX Health & Science Web Store, please fill out the return form below. Any product returns must be requested within 30 days of purchase date or a re-stocking fee will be assessed.

If you are returning an Upchurch Valve, please print out the Valve Return Information Sheet and return it with your Valve. Please note: You will still need to contact Customer Service prior to the return.
